Issuance of certificates

  Certificates cannot be issued on the same day, so please apply in advance. 

 If you need documents such as a medical certificate, medical opinion letter, or hospitalization confirmation before discharge, please inform the nursing station 2–3 days prior to discharge. 

 Documents cannot be issued without valid ID and required paperwork (in accordance with Article 13, Paragraph 2 of the Enforcement Rules of the Medical Service Act, Ministry of Health and Welfare Decree No. 158). 

A designated fee must be paid upon issuance.

In the event the patient is missing

  • Document confirming the fact that the patient is missing, such as a resident registration or a copy of the court’s declaration of disappearance
If the patient is legally incompetent
  • ID or a copy of the person requesting record access or copies
  • Document verifying family relationship, such as a family relationship certificate or resident registration
  • Certificate of guardianship registration or a psychiatrist’s medical certificate proving the patient’s legal incompetence
